The CPM program provides a comprehensive, individualized health examination to thoroughly assess each person’s current state of health, screen for early signs of disease, and identify ways to decrease the risk of future disease in an efficient, organized manner. When you visit the CPM, you will receive:

• A full-day, comprehensive health exam, tailored to address your specific health status, objectives  and concerns

• Same-day test results and wrap-up consultation with an internist
• Attention to disease prevention
• Assessment of fitness and nutritional status, evaluation of lifestyle behaviors, and personal goal setting
• 12 months of follow-up coaching on nutrition and fitness, tailored to each individual

Primary Care

For individuals interested in continuing their highly personalized healthcare experience, CPM offers membership in the concierge service, Program 24/7.  This membership was designed to help busy individuals navigate the increasingly complex healthcare system by providing expedited access to their primary care physician, facilitation of appointments with top specialists, elimination of long waiting times and coordination of care. As Program 24/7 members, individuals may take advantage of premium services, including:

• Guaranteed same- or next-day appointments with their CPM internist or the designated internist on call
• Physician availability 24 hours a day, 7 days a week
• Assistance scheduling and coordinating appointments for follow-up or diagnostic studies
• Coordination of care for diagnosis or treatment by multiple providers
• Referrals to top specialists
