How to Become an Explorer

If you are interested in becoming a medical explorer and would like to apply, please submit a completed application and the following items:

• $50 annual registration fee, which includes all on-site, hospital-sponsored activities, as well as uniforms (Fee Waiver with proof of free/reduced lunch program)
• Official school transcript
• Personal statement
• Academic recommendation from your school counselor or teacher
• Parent/guardian signature on the Northwestern Memorial Hospital Medical Explorer Standards and Bylaws document, which you can print out, sign, and send to us

Additionally, you must be at least 14-years-old and must have completed 8th grade by the time you begin the program.

Medical Explorer Membership Application

The Medical Explorer membership application process is closed for the 2011-2012 school year. Please watch for next year’s application in summer 2012.
