Community Matters Programs
Over the last few years, Northwestern Memorial, Erie Family Health and Near North Health Service Corporation have focused on two primary strategies:
- Identifying priority health concerns within the community and developing formal, comprehensive initiatives aimed at responding to these conditions
- Working together to systematically reduce known barriers to accessing all patient care services, especially for low-income, African-American and Hispanic persons
Together we have developed models of care for chronic disease that have a measurable, positive impact on the health of medically underserved residents in our communities, including:
Women's Health Initiatives
Our women's health initiatives help at risk women access everything from pregnancy and gynecologic care to screenings for breast and cervical cancers.
Diabetes Collaborative
Our diabetes collaborative helps diabetic patients living in underserved areas.
Homelessness & Mental Health
We work with the Chicago Alliance to End Homelessness through programs aimed at identifying homeless people with mental illness, and providing housing, social support services and medical and mental healthcare.
Food Oasis Program
Walgreens, Northwestern Medicine and Near North Health Service Corporation are partnering together to pilot the "Food Oasis" program, a pilot designed to encourage healthier food choices for patients coping with chronic illness, like diabetes.
