Our Dedication to Diabetes Care
Since 2006, the Northwestern Memorial Diabetes Collaborative has been helping patients living throughout the Chicagoland area better self-manage their diabetes.
This is a unique partnership between Northwestern Memorial Hospital, our community health partners Near North Health Services Corporation and Erie Family Health Center, and Northwestern University Feinberg School of Medicine.
The Northwestern Diabetes Collaborative is one of five programs in the nation to receive the prestigious Nova Award by the American Hospital Association (AHA) for their hospital-led efforts that improved community health.
The collaborative is dedicated to providing trusted health information on diabetes management and prevention, and incorporates the following into patient care:
- Culturally sensitive patient education
- Videos and print materials in Spanish and English
- Self-management skills training
