Heart Transplantation Case Managers & Medical Directors
Many patients transfer to the Northwestern Memorial's Center for Heart Failure from outside institutions for heart transplantation. We recognize the integral role that case managers and medical directors play in coordinating a safe and seamless transfer of care for their patients to the Center for Heart Failure. Our goals is to work with case managers and medical directors and provide the necessary resources and information to best help their patients.
Northwestern University Affiliated Transplant Centers
The Bluhm Cardiovascular Institute and the Kovler Organ Transplantation Center of Northwestern Memorial Hospital are part of a joint endeavor with the transplant program at Children's Memorial Hospital. Together, these three entities are known as the Northwestern University Affiliated Transplant Centers (NUATC).
NUATC was established in 1997 to create an environment in which patients of all ages can receive outstanding comprehensive transplant care, where case managers and medical directors from outside institutions play an integral role in coordinating patient care, and where physicians and surgeons can contribute meaningfully to the understanding of transplantation through research.
