Medical Records

The Medical Records Department can help you obtain a copy of your medical records. To request your medical records, please complete the following steps:

  1. Download the Authorization for Release of Information (pdf)
  2. Mail the completed form to:

    Northwestern Memorial Hospital
    251 East Huron Street
    Medical Records - Customer Service
    Feinberg/2nd Floor/2-304H
    Chicago, Illinois 60611-2908

Please note: Only Northwestern Memorial Hospital patients can request records by FAX. Please download the Authorization for Release of Information (pdf) for details.
