Diabetes Collaborative Honored for Improving Community Health
Partnership receives prestigious NOVA award from the American Hospital Association
Northwestern Memorial Hospital’s Diabetes Collaborative has been recognized as one of five 2011 NOVA Award winners from the American Hospital Association (AHA). AHA NOVA Awards are presented each year to a select group of recipients in the hospital field that can demonstrate collaboration and an improvement in a community health status—whether through healthcare, economic or social initiatives.
The Diabetes Collaborative represents the partnership between Northwestern Memorial Hospital, Northwestern University Feinberg School of Medicine, Near North Health Services Corporation, and Erie Family Health Center. The collaborative showcases an educationally based, holistic approach to managing diabetes. With $2 million dollars in grant funding from Northwestern Memorial over five years, the comprehensive program leverages evidenced-based care and Chronic Care Teams to create multimedia tools that teach adult patients with Type 2 diabetes how to better manage their chronic disease. Information includes seven culturally sensitive education videos, print materials in Spanish and English and other tools including:
- Patient education and self-management skills training
- Evidence-based clinical practice guidelines and provider education
- Clinical information systems
- Feedback mechanisms offering performance metrics on chronic illness measures
- Delivery system design changes
The American Hospital Association (AHA) will honor five programs for their hospital-led collaborative efforts that improved community health in an awards ceremony to be hosted on July 19 during the Health Forum/AHA Leadership Summit in San Diego. The winning programs are The Diabetes Collaborative in Chicago, Emergency Department Consistent Care Program in Olympia, Wash., Integrated Community Nursing Program at Parkview Health in Fort Wayne, Ind., Milwaukee Health Partnership, Milwaukee and Glendale, Wis., and the Rochester Youth Violence Partnership in Rochester, N.Y.
