Northwestern Memorial Hospital Palliative Care has three linked programs available to meet the full range of healthcare needs for those seeking palliative care:

Supportive & Palliative Care Clinic

We offer outpatient assessment, consultation and treatment for patients and families who are dealing with issues like symptom management and advanced planning. The clinic staffs physicians, nurses, pain specialists, social workers, chaplains and volunteers who are experienced in alleviating physical pain, assisting with emotional and spiritual matters, and future planning.

Palliative Care Consultation

Physicians, nurses and social workers consult with patients and their caregivers in the hospital about issues of symptom management and advanced planning. Consultations focus on helping patients and family members understand their care and treatment options so they can make informed decisions.

We are experienced in managing many of the symptoms that accompany serious illnesses including pain, shortness of breath, fatigue, constipation, nausea, loss of appetite, and problems with sleep, among others. It can also help you manage the side effects of the medical treatments you’re receiving.

Palliative Care Inpatient Unit

Our inpatient unit is staffed by physicians, nurses, social workers and other healthcare specialists devoted to relieving symptoms associated with life-limiting illness and helping patients and family members plan their future.

End of Life Care & Hospice

When an illness has progressed to an advanced stage, treatment to relieve pain and other symptoms continues. The palliative care team will remain closely involved with a focus on optimizing comfort, alleviating suffering, and supporting patients’ and families’ decision-making whether or not cure is still possible.

Individual & Family Discussions to Clarify Goals of Care

The palliative care team encourages an open dialogue between the patient, family and medical team. The purpose is to help the patient identify his or her personal goals and wishes, to provide a well-planned, complete treatment for all of your symptoms throughout your illness.
