To refer your patient to the Parkinson's Disease and Movement Disorders Center, please download the rapid referral form (pdf). This file contains a brief demographic page about the patient as well as the Florida Surgical Questionnaire for Parkinson Disease. Please include the materials requested on the form as well as the following information:
- Recent office notes
- The patient's current medication regimen
- Recent imaging reports
- Any specific questions, concerns or requests
