TJC: The Joint Commission
The Joint Commission is the organization that accredits Northwestern Memorial Hospital and thousands of other healthcare organizations. The Joint Commission has developed and adopted quality measures that hospitals voluntarily collect from patients’ medical records. You may view the data on their web site.

- Prevention of blood clots in stroke patients
- Medication given to ischemic stroke patients at discharge to prevent recurrence of stroke

- Medication to prevent blood clots for stroke patients timely
- Cholesterol reducing medication prescribed to stroke patients at discharge
- Education for stroke patients and families provided
- Evaluation of stroke patients for rehabilitation care
