Your evaluation is an important first step in the stem cell transplant process. The following items may help make your visit more efficient:

• Bone marrow biopsy results and slides
• Lymph node biopsy results and slides
• CT report
• X-ray reports
• MRI reports
• PET scan results
• Chemotherapy history and dictation from your physician
• Any lab results
• Insurance Information
• A family member or companion
• A list of questions you would like to ask the physician or other team members
