Becoming a Patient

The following information will help you prepare for your hospital stay.

Schedule a Procedure
Your doctor's office will provide you with an Outpatient Registration Form that will list the phone number for you to call to schedule your appointment.

After scheduling your appointment, call our Patient Access Unit at (312) 926-2950 or (888) 272-0166 between 8 a.m. to 5 p.m. Monday through Friday to provide patient information:

  • Name
  • Date of Birth
  • Social Security Number
  • Insurance provider information
  • Employer information
  • Your doctor's name
  • The medical reason for your visit

Arrive at the hospital
On the day of your appointment, report to the reception desk on the first or second floor of Feinberg Pavilion, 201 E. Huron. A Patient Liaison will be assigned to you and escort you to your room.

What to Bring

  • A list of medications you are taking.
  • Medical information or records (including X-rays) from your personal physician, if possible.
  • Your medical insurance information.
  • Your own nightgown or pajamas, robe, slippers, toiletries, reading glasses and a small amount of cash, if you wish.
  • Please do NOT use your own heating pad, heater, heating blanket, curling iron, VCR or television set.
  • Please do NOT bring valuables with you. The hospital is not responsible for the loss of money, jewelry or other valuable personal belongings.
  • Laptop computers are allowed, but do not fit in the room safe and may be left unattended during your hospital stay

Going Home – Discharge Procedures
When it is time to leave the hospital, your attending physician will write a discharge order. A nurse will provide instructions about any special needs, such as medication requirements, you may have after leaving the hospital.

Discharge time is 11:00 a.m. Before leaving your room remember to do the following:

  • Review your discharge guidelines with your doctor or nurse, and ask questions you may have about your home care
  • Let your nurse know if you need assistance with packing
  • Have your prescriptions, follow-up appointments scheduled and referral information
  • Remember to take any equipment or supplies provided to you by your therapists
  • Talk to your nurse about reclaiming valuables left with security

Pick-up
When your ride arrives, they may use the Galter drive-through, entering from Huron Street, without paying parking fees.

Patient Survey
We want to know what you think about the care you received at Northwestern Memorial to help us learn how we can better serve our patients. Shortly after you return home, you will receive a survey in the mail. Please take the time to complete and return it in the postage-paid envelope provided.
