Northwestern Memorial Hospital Palliative Care and Home Hospice Program strives to provide the best patient and family experience by enhancing the dignity and quality of life of those facing life-limiting illnesses. We aim to achieve this by addressing the physical, emotional and spiritual needs of patients and families with an interdisciplinary team of dedicated professionals and volunteers.
